What is a Pacemaker and why is it required?

A pacemaker is a device that is designed to provide an electrical signal to the heart muscle and help maintain a proper rhythm. There are a number of pacemakers and the particular model selected is based on the specific condition of the patient, but all pacemakers share a common design. The pacemaker will consist of 2 major pieces: a small metal box that contains a battery and other electronic components and an insulated wire called a lead which then carries the electrical impulses up to the heart.  The pacemaker will be permanently implanted in your chest and depending on your condition either one or 2 leads will be attached to the heart muscle.

When does an individual require a pacemaker?

The heart is located in the center left of the chest enclosed within the breastbone and rib cage. The rhythmic contraction causes the blood to circulate. The contractions of the heart are triggered by a tissue called the SA node. The SA node generates a small electrical signal that is transmitted by nerves to the surrounding muscles. These electrical impulses are what caused the heart muscle to contract. In some cases the SA node fails to cause the heart contraction causing an abnormal heartbeat or arrhythmia. The most common form of arrhythmia for which pacemaker surgery is often recommended is Brady arrhythmia or slow heart rate. There are a number of reasons why you may have developed an arrhythmia but in most cases the problem is caused by disruption in the SA node or in the system of nerves that conduct electrical signals to the heart muscle.

What is the procedure?

The patient will be administered with general anesthesia, and then the surgeon will make a small skin incision in the upper chest just below the collarbone. A pocket is created between the skin and the tissue that covers the chest muscle.  The doctor then uses instruments called retractors to hold back the skin and underlying tissue to locate a large blood vessel called the subclavian vein. Using a special needle and syringe your doctor will puncture the wall of the vein guide wire and into the vein the doctor gently pushes the wire until it reaches the heart using an instrument called a fluoroscope. Once the wires are in place, the needle is removed and a catheter or hollow tube is passed over the guidewire and enters the heart. The leads are then passed through the catheter. When the lead or leads are placed at the desired place then the catheter is removed. Finally the leads are then connected to the pacemaker, and the pacemaker is inserted into the pocket below the collarbone and then the incision is closed but for a successful surgery one needs the pacemaker surgery hospitals in India.

What are the risks post the surgery?

As the procedure is extremely simplified because of the use of modern technology and skilled surgeons. But even then it can pose a few risk factors. The most common of the risk factors associated include:

·       The patient can develop allergic reaction to the anesthesia

·       Blood clots, bleeding and nerve damage can be a very common risk post the operation

·       Infection at the site of surgery due to the incision is common

·       Low blood pressure experienced by the patient post the surgery is extremely dangerous

·       There can be a buildup of the scar tissue around the pacemaker over the time
